Active Ageing Statement 2016

Sirona care & health Community Interest Company (CIC) is committed to providing joined up health and social care services that support individuals in their communities and is NHS and Local Authority funded. Sirona launched the ‘Active Ageing Service’ in South Gloucestershire in April. It service aims to enable older people to maintain independence and to promote dignity and quality of life in their own homes by offering health and social advice. The Active Ageing Service aims to offer a visit to everyone between the ages of 80- 84 years. The team consists of Health Visitors and Health Visitor Support Workers who are based in the community. We are able to help people access services, such as medical, social, home environment and financial. We also support carers.

The Service Offers

  • Falls and accident prevention
    • Advice on memory loss and dementia
    • Support for those who have been recently bereaved
    • Advice on managing long term health conditions
    • Health education
    • Signposting to local and national support services

We work in partnership with your GP and other health and social care

Prescription Hub

 

As of the 1st November 2021 all prescriptions for Streamside Surgery practice will be dealt with by the new PCN Prescription Hub – Severnvale Prescription Hub.
